Various payment plan options effective 03/01/2022
Credit Card or ACH option:
Once option is chosen, payments may be made by mailing check, cashier’s check or money order You may also visit our website, acfmins.com and pay by Credit Card or ACH. It is fast, simple, and secure! To use the credit card option, you will be charged a fee of 3.25% of the amount paid for each transaction. To use the ACH option (Automatic Clearing House – deducted directly from your bank account) you will be charged a transaction fee.
Pay the Annual Premium in full. Pay the annual premium as shown on the enclosed notice by mailing check, cashier’s check or money order. Payment must be received at ACFM on or before the due date.
Semi-annual installment payment plan option. Should you elect the semi-annual installment payment option, your first payment for 50% or 1⁄2 of the billed premium, plus a $4.00 service fee must be received at ACFM on or before the due date shown on the premium notice. You will be billed for the remaining balance at the end of the first six (6) month period. The second installment amount plus a $4.00 service fee must be paid and received at ACFM on or before the due date on the enclosed notice.
Note: The payment options you may use to pay by credit card
and or ACH are the same as in Option 1 or Option 2 above
